[request] plugin that creates portals to other servers

hello first wanted to mention that I'm from Brazil so my english is not very good
and what i precuro and a plugin that create portals to get into other servers who know what makes this plugin please pass me hoping he answers

 

Google bungee-cord or lily pad. It's not really a plugin, but you can use it to connect to other servers. I don't think its poosible with ONLY a plugin.

 

This would require a client mod as well as the plugin, unless you use bungee-cord+spigot.
