{PermPrefix} {group} In front of name instead of The prefix and the group Help!

So basically, something went wrong and now we have this problem:

And need to know how to make it so instead of: {PermPrefix}{group} NAME
We get [Member] NAME. Also, a side issue, we get [Member]Member, because it re-states the group name, and I was wondering how to remove it, thanks!

ALSO: Permissions are not working AT ALL now. I can't even do /help, when before I could. And if I type /op I can op anyone, when I can't even do /help... any help is appreciated! Were desperate D:
